Hepato-lienal fibrosis without cirrhosis: non-cirrhotic intrahepafic portal hypertension

چکیده
In 1933, John McMichael was awarded a gold medal by the University of Edinburgh for his M.D. thesis. This was entitled 'The pathology of hepatolienal fibrosis' and was published in an abridged form in 1934 in the Journal of Pathology and Bacteriology. It was a masterly account of the pathological changes associated with portal hypertension. Pathological changes in spleen and portal vasculature were identical whether or not cirrhosis was present. McMichael was particularly interested in the non-cirrhotic cases. In all of them a mild 'hepatitis' usually portal zone lesions, could be demonstrated. He speculated that it might be due to vasodilatation of the hepatic arteries or to spasm of the portal venules in the liver. These observations were at least 30 years ahead of their time. The newer techniques of intrasplenic and wedged hepatitic venous pressure measurement, hepatic blood flow and splenic venography have only served to confirm McMichael's original observations (Fig. 1).

متن کاملPlasma Prothrombin Time and Esophageal Varices in Patients with Cirrhosis of Liver
Introduction Cirrhosis of the liver is a common complication of chronic liver disease and is associated with portal hypertension and esophageal varices. In this study, we checked the implication of prothrombin time, if any, in the genesis of esophageal varices. Materials and methods Sixty patients with cirrhosis of the liver were randomly assigned into two groups: Group I - 30 cirrhotic patie...
متن کاملDistal pancreatectomy with splenorenal shunt to preserve spleen in a cirrhotic patient
At pancreatic ductal adenocarcinoma is an aggressive malignancy with a high recurrence rate. Due to its high potentials of local invasion and distant metastasis, surgical resection is the only means for possible long-term survival. Surgical treatment comprises a distal pancreatectomy with or without splenectomy. Surgery has been conventionally contraindicated for patients with cirrhosis and por...
متن کاملGastric Varices in Absence of Splenic Vein Thrombosis: A Rare Entity of Idiopathic Non-Cirrhotic Portal Hypertension
Idiopathic non-cirrhotic portal hypertension (INCPH) is portal hypertension (PHT) without cirrhosis and other identifiable causes. Esophageal and gastric varices are seen in INCPH which are mostly asymptomatic. We present a rare case of symptomatic isolated gastric varices (IGV) in the setting of INCPH. We report a case of a 60-year-old man who presented with an acute onset of hematemesis and n...
متن کاملRelevance of plasma malondialdehyde level and severity of portal hypertension in cirrhotic patients.
BACKGROUND Portal hypertension is one of the death reasons for the liver cirrhosis patients. The oxidative stress is related to the occurrence and development of portal hypertension in cirrhosis. Malondialdehyde (MDA), one of the lipid peroxides, increases substantially in cirrhotic patients. AIMS To evaluate the relevance between the MDA level and portal hypertension in cirrhotic patients. ...
